Factual error: In the opening sequence, Voyager is passing through a gas cloud. In this sequence Voyager is parting the gas and dust like it would if there was air around, there even are some turbulences near the warp nacelles. Voyager utilises a navigational deflector, which effectively cleans space in front of Voyager. The CGI in the opening sequence shows this only to be a few metres in front of Voyager's bow. How ever navigational deflectors form overlapping shield bubbles a few hundred meters in front of the ship, even at low impulse speed. Flying through such a cloud would even require engaging combat shields, which would form a bubble around Voyager bigger than leaving a few meters of space between the shields and the hull. Even if the shields weren't conformal to the hull there would be more space. Last but not least the navigational deflector would provide particles with a directional impulse straight away from the ship, and not letting it slide along the deflector. The displayed turbulences near the nacelles would be highly abrasive to the ship's hull.

Continuity: Tuvok and Paris' rank seems to skip around during the first season. Despite being listed in the credits as 'Lieutenant Tuvok', Tuvok starts the show as a Lieutenant Commander, with the signatory three collar pips, one black and two gold. At certain points, however, he is seen with only two gold pips (signifying Senior Grade Lieutenant). After season one he remains a Lt. Commander with no reason for the lack of continuity offered. Similarly, Paris starts the show as a Senior Grade Lieutenant, but sometimes instead of two gold pips he has one gold and one black, the signifier of a Junior Grade Lieutenant. After season one he is always presented as a Junior Grade, excluding his brief demotion to the rank of Ensign in a later series.

Revealing: If you look carefully when Neelix shoots the bins of water causing the water to rush out you can see a faucet opening on one of the bins.

Continuity: The registry on the shuttle that brings Paris to Voyager keeps changing between "NCC 71225" and "NCC 1701-D".

Continuity: Just after Voyager is hurled across the galaxy, Captain Janeway and the crew are recovering from the impact. Janeway's hair, done in an updo bun, has become disheveled and strands are hanging loosely. Briefly after, when she's in engineering, her hair is neatly coiffed again; the hairstyle is too complicated for her to have so quickly and while under duress have placed the strands back so perfectly .

Plot hole: To escape the 'event horizon', a rupture in it is widened to 120 meters (which allows 2 meters clearance on each side of the ship). Soon after, the rupture has shrunk to 110 meters wide, and Janeway decides to 'punch through' anyway. The ship does so with little difficulty, but the whole issue seems pointless since the rupture was far larger 'vertically' than 'horizontally' (relative to the view from the ship), so tipping the ship on its side would have allowed plenty of clearance to slip right through.

Plot hole: The Voyager was continuously attacked by small Kazon attack ships. Yet the Captain never orders to increase the warp speed to evade them (In previous episodes we saw that the Voyager has much higher max speed than any ship in Kazon arsenal). They simply travel at a lousy Warp 2 and wait to be attacked. It would make sense to travel slowly if they avoid being detected by Kazon but that was not the case. They were attacked constantly. Voayger may be more maneuverable at slower speeds but if you can outrun your pursuers there is no need to maneuver to avoid them. Also, if Voyager was traveling at a higher Warp Speed, there would be no way for the Kazon to array fighter craft ahead of Voyager as they would not be able to overtake Voyager. If they could, there would be no reason to deploy the fighter ships, they could simply engage with their capitol ships.

Continuity: In the second part of this episode, the Doctor asks the ship's computer of how many persons are on board to get an idea of the number of invading Kazon he is facing. The ship responds with "89 Kazon and one Betazoid". It fails to mention Seska who is a Cardassian.

Continuity: As Starling prepares to launch the timeship from the bay behind his office, there is a shot of the exterior of the ship. In the background of this shot, there is a white wall on the right. This wall carries the company name and logo. However the name is misspelled here as Chronowerks, instead of Chronowerx as was shown on the outside of the building and behind Starling and Janeway when they were in the office.

Visible crew/equipment: The tip of the boom appears briefly at the end when Janeway and Chakotay are in the Captain's ready room.

Revealing: When Tom is reading the Klingon romance novel, he presses the button to turn the page, yet the text is always the same.

Factual error: When Janeway visits Seven as she works to destroy the Omega molecules in the chamber, Seven reports that 11% have been neutralized. They then have a 1 minute, 10 second discussion, and when it's over, Seven states that 18% of the molecules have now been neutralized. Janeway comments, while checking the computer screen, that at the current rate, "this could take hours". Actually, at the current rate, it will take less than 15 minutes.

Continuity: Early in the episode, the Kyrian historical researcher plays a simulation of events aboard Voyager that include Janeway killing a Kyrian and then throwing the rifle to a crewman. Later, he replays the simulation for the Doctor, but subtle details of this 'same simulation' change, most notably the way the female crewman raises her rifle much sooner the first time we see Janeway walk past her, and the difference in the way the male crewman catches Janeway's rifle.

Continuity: Paris reports that several ships have dropped out of warp off of Voyager's port bow, but when the ships are put on screen, they are approaching from starboard aft.

Continuity: When Tom explains to Alice why he can't leave the Voyager, he has a three-day beard. Later in the ship, when Alice convinces him to turn on the neurological interface, he is cleanly shaved. But in the next scene he has his beard back again.

Plot hole: Tuvok says that he was born on Stardate 38774. But he was born in 2264, when Stardates were only counted in the thousands, not the tens of thousands. If he were really born on SD 38774, that would mean he was born only a few years before the Enterprise-D was launched.

Continuity: When Neelix shows Torres her own quarters, her bat'leth can be seen in the background. It's a run of the mill bat'leth, unlike the one she actually hung up in 'Prophecy', which was gunmetal and had a spike in the middle.

Audio problem: When Chakotay comes into Seven's quarters at some stage, the comm badge sound is used, not the standard door chime sound.
