1	The Simpsons	This is a mistake for the introduction for the later episodes. When Homer screams, he turns round. You see this in a wide shot. There are no boxes to the right of the door in the garage. However, two just appear out of thin air when he runs through the garage.
2	Friends	When Joey is counting the floors of Ross' building (the first time) to find out where Hot Girl lives, he is wearing a black top with a zip. When he rushes over to Ross' building he walks down the hall and knocks on what he believes to be Hot Girl's door. In these shots he is wearing a dark purple button up shirt. As soon as Ross answers the door, Joey is wearing the black top with a zip again.
3	Sex and the City	At the beginning of every show we see Carrie walking down the street and she gets splashed by the bus. First the bus has people on it, then in the next shot they all disappear.
4	Buffy The Vampire Slayer	When Giles is giving his speech about the Hellmouth the books Buffy's holding keep changing from facing him to facing Buffy and back again.
5	24	When Nina's just been shot, and makes it to the barn to call CTU (around 7.10am), while she's speaking on the phone there's a cameraman visible on the right hand side (DVD).
6	Lost	When Hurley is at the new house, after his mom hurts her ankle, he reaches into the car window to get the cell phone. When he begins to dial, the cameraman's arm moving about is reflected in the back chrome of the side view mirror.
7	Family Guy	Brian's newspaper's name changes from "Daily Informant" to "Daily Tim...". The newspaper also turns upside down between the two shots.
8	Scrubs	The woman in the wheelchair that Dr. Cox claims is dead is wearing glasses in some shots but not in others.
9	Smallville	Clark's hands on the hood of Jeremy's truck shift in different shots from spread wide to in front of him and close together.
10	Red Dwarf	When Rimmer leaves for his exam, he picks up a case from the seat by the window. When Lister locks the door and goes over to the cupboard, the case is still on the seat.
11	Angel	In the last scene of the episode, Angel kicks the guy out of the glass building. This guy's a vampire, and you can see his reflection in the glass windows. Vampires aren't supposed to have reflections.
12	The X-Files	When Mulder and Scully are questioning Lauren in the investigation room, the piece of paper on Mulder's jacket changes place in between shots.
13	Mr. Bean	In the later intros (when Mr Bean falls into a spotlight in a town) part of Mr Bean's foot isn't in the spotlight, but it still has a shine.
14	The Blue and the Gray	During the Battle of Bull Run, just before the first shell is fired towards the plantation house, at the upper part of the screen you can see a helicopter flying over the battle action.
15	The O.C.	When Sandy first picks up Ryan, we see in several shots that Ryan's bike is in the trunk, and that the trunk is open. However, when they drive up to the house, the trunk is now closed, with no bike.
16	Charmed	When Phoebe goes to sign her papers at the end, she spells her name as Pheobe.
17	Will & Grace	Ellen and Rob wear wedding rings, yet their wedding is not announced until "An Affair to Forget" in the second season.
18	Stargate SG-1	Carter and Cadet Haley enter the Stargate and we see them through the "puddle." When they come out on the other side, their positions are reversed.
19	Stargate: Atlantis	When they first arrive on Teyla's planet, the DHD is close to the gate. When the wraith attack, the DHD is much further from the gate.
20	Futurama	When Fry falls into the cryogenic chamber his whistle drops right next to the chair. When it cuts, the whistle is gone.
21	Heroes	S02E09 - When Claire runs away from Bob after he calls her "Ms. Bennet", you can see Bob's shadow and another person's shadow on the ground. Since no one else was standing next to them, it has to be a crew member, most likely a camera man based on the odd shape of the shadow.
22	Xena: Warrior Princess	As the people of Troy are about to be attacked by the warriors in the horse, the camera pans up to the side of the wall where Deiphobus is talking with another soldier. You can see a crew member crouching down behind them wearing a white and grey shirt.
23	Frasier	When Frasier and Niles are eating the steaks at the restaurant that Martin brought them to, in some shots you can see Frasier's baked potato, and in others from the same angle it is not visible.
24	Seinfeld	When George is lying on the floor in his underwear, a newspaper is on his right side. When they take the reverse shot (Jerry's view of him lying there), the newspaper is on George's left side.
25	King of the Hill	After Hank and Bobby return from La Grunta, Hank parks the car halfway into the garage. Yet when Peggy washes the blood from the deer off the front of the truck, the truck is now on the driveway and not near the garage.
26	Battlestar Galactica (1978)	When Baltar turns to face the two setting suns, we see the lower one at the left has what could be a boat in its reflection on the water. The higher one on the right is over a mountainous area on the horizon. The next shot is of Baltar in close-up, then the next returns to the horizon to show us Baltar's view, which shows just ONE setting sun. The one on the left has vanished though there is room enough for it to have been seen in this shot.
27	Desperate Housewives	Episode 2:20 - While Susan and Edie are at the Hooters style restaurant, the amount of beer, and amount of head, in Edie's glass changes from shot to shot; It's not because she is drinking it.
28	Doctor Who (1963)	After UNIT HQ disappears, you can see a patch of grass where the UNIT HQ used to be...but just exactly how can grass grow underneath a building?
29	M*A*S*H	When Radar and Burns leave the tent for morning parade, Radar's clipboard acquires a pencil.
30	Alias	When Syd's Dad rescues her in the car park, he starts shooting at the other car. After firing one shot the slide of his gun locks back (indicating it's out of bullets), but when the angle changes he carries on firing with no problems.
